If you are applying for individual health cover, the next step you will be taken to be filling out the forms that are required for the application process. Now, when you are done with completing your application, you should click on the "Submit" button. As previously stated, if you are completing a Medicare or Medicaid application, you will be prompted to complete a Medicare Supplement Eligibility Certificate. In the case of applying for private insurance, you will be taken to a page where you will need to complete a medical release form.
