Now, when you are done with completing your application, you should click on the "Submit" button. As previously stated, if you are completing a Medicare or Medicaid application, you will be prompted to complete a Medicare Supplement Eligibility Certificate. In the case of applying for private insurance, you will be taken to a page where you will need to complete a medical release form.
